The health center uses a health care team model that includes three physicians, one nurse practitioner, one physician assistant, and two social workers, and nursing staff that are all genuinely committed to providing excellent health care. They also have a care management team, an outreach department, and a dedicated administrative staff that bridges the gap between patient and provider.
